This is a resource that I commonly use for patients that I transition to a restorative nursing program after discharge from therapy. It can also be used during cognitive treatment as a planning tool when a patient is receiving multiple therapies . The possibilities are endless!
I find when using this resource the patients and staff are more accountable when transitioning to a restorative nursing program. There is also a sense of independence the patient demonstrates by creating their own schedule.
Use: I like to laminate everything prior to use to increase durability. The different therapies/activities are then cut out individually. I then place a piece of tape or Velcro on the back of the schedule pieces so the patient is able to change items on a week to week basis, or however often is needed. Hope you enjoy!
