Communication with the kitchen is so important for a successful workplace and overall patient care. During treatment, I will sometimes order a “trial tray” along with other therapeutic trials prior to recommending a diet change. I’ve found the trials can lead to possible misunderstandings about diet changes before I recommend an upgrade/downgrade or modification. To prevent this from happening, I use this trial tray form which lists the current diet, textures to be trialed, and during which meal. I’ve included different variations of this form with the IDDSI and NDD model to accommodate all settings. Hope you find this helpful!
