Have trouble remembering the diets of everyone you see? Looking for an additional way to communicate diets with staff? This resource includes 5 different types of logs to document diet recommendations and/or the patient’s current diet. I travel to multiple buildings during the week, so I find this resource helpful to keep track of diets and with staff questions. I also place one of these documents in the therapy gym so my rehab team is up to date on all diets including upgrades/downgrades. The nursing staff also finds this helpful when I provide them with a resource to refer to for diets and recommended methods for administering medication. New staff hires appreciate this resource when getting to know patients, because as we all know when starting a new job it can feel like information overload!
As I mentioned, I travel to multiple building and not everyone goes by the same diet levels (IDDSI, NDD, etc). I’ve included a log with an IDDSI and NDD key at the bottom to accommodate all settings. A “Notes” column was created to document any strategies, maneuvers, precautions, or any additional information to help staff! The “Date” column helps to indicate the most up to date diet if any changes occur. I find that the more communication and resources for diet recommendations, the more consistent carryover will be.
