This download includes multiple variations of a visual support to assist with orientation. Often times, I use this for patients with memory deficits or increased confusion due to various reasons. Bright colors were used to help stand out and visually locate.
A line is included at the top of each sign to type in the individual’s name or date. Various topics are listed covering safety and temporal information. Each page is editable so information specific to the patient can be typed into the document.
The documents can also be printed as is and then laminated in order to be used with a dry erase marker to write daily information.
